Privacy
The information received and collected about our clients/patients from their visit is strictly private and confidential. It is used and viewed only by the healthcare professionals and staff employed by this clinic, unless, in the best interest of the client/patient, the practitioner determines that there is a need to communicate with another person or healthcare professional outside of this clinic. We will not give, share, sell, or transfer any personal information to a third party unless required by law. Under absolutely no circumstances would this communication happen without the signed consent of the client/patient.
If you are going to be more than 5 minutes late, please call to let us know. A 24-hour notice for cancelled or rescheduled appointments is required in order to avoid the cancellation fee equal to the scheduled service fee. If you are able to reschedule within the same week, the fee may be applied to the rescheduled visit. Missed appointments are the patients responsibility and are not covered by Insurance. Your notification allows us time to schedule another patient that would also benefit from treatment. This appointment policy allows us to develop a mutual consideration and respect for your time and ours.
Appointments
Payment is due at the time of service. We accept cash and all major credit cards as well as Health Savings Accounts.
If you have insurance and your insurance plan includes *Out-of-Network benefits for Acupuncture services*, we are happy to verify your benefits and submit claims to your insurance company on your behalf. Once your claim is processed, the insurance payment is received by Sacred Fortune, and all balances are satisfied minus any deductible or copays, reimbursement will be applied to your account as a credit or refunded. Because insurance plans vary, coverage and reimbursement amounts cannot be guaranteed.
